Participant Waiver (check box)

I acknowledge and voluntarily agree to the terms below as a participant in the Wedding Ready program offered by Wedding Ready, LLC:

I understand that the program includes physical fitness exercises, nutrition advice, and related activities.
I am aware of the inherent risks involved in physical activities, including the risk of injury or illness, and I take responsibility for my own health and safety during the program.
I assume all risks associated with participating in the program, including those related to physical injury, illness, or health conditions, and agree to consult with a medical professional before starting any exercise or nutrition program.
I release Wedding Ready, LLC, its owners, instructors, employees, and affiliates from any claims, liabilities, or actions arising from my participation in the program, including those related to personal injury, property damage, or death.
I understand that I am responsible for any costs associated with necessary medical treatment or services resulting from my participation in the program.
I grant Wedding Ready, LLC permission to use photographs or videos taken during my participation in the program for promotional purposes, both online and offline, and I acknowledge that my name and identity may be associated with such media.
I confirm that I have read and fully understand the terms of this waiver and release form, and I am voluntarily agreeing to it with full knowledge of its significance.
